Johnson & Johnson (JNJ) stock returned 59.9% over the past year, against 21.1% for the S&P 500. The company has a clean story for it. The biosimilar hit it had been bracing for arrived and was absorbed, and its talc litigation now has a proposed ending. Both are real. The first does not explain a move that size, and the second is not finished.

Johnson & Johnson Lost More Than Half Of STELARA And Still Grew

STELARA sales fell 55.7% in Q2 2026 as biosimilars took share, a drag of roughly 460 basis points on the whole company’s growth. Johnson & Johnson still reported operational sales growth of 5.6% in the quarter, and management said that excluding STELARA the company grew double digits. TREMFYA, the immunology drug it is pushing into ulcerative colitis and Crohn’s disease, grew 71%.

Management raised the full-year 2026 operational sales outlook to 6.5% to 7.1% – a midpoint of $100.6 billion – growth that includes about 100 basis points from a 53rd week in the financial calendar. The company is on track, it says, to pass $100 billion of annual revenue for the first time in its 140-year history.

So What Else Was Doing The Lifting?

Revenue over the trailing twelve months is $97.93 billion, up 8.1% year over year, against a three-year average of 6.0%. That acceleration is real, and small. Reported profitability went the other way: net margin sits at 21.5% against a three-year average of 29.4%. That baseline, however, was heavily skewed by below-the-line items—primarily the one-off gain from the Kenvue separation—which is why the historical net margin sits above the three-year average operating margin of 25.7%. With operating margin currently running at 26.8%, core business profitability has held steady; the apparent margin drop reflects normalization after that windfall, not operational erosion.

The operating news was not uniformly good either. Abiomed sales fell 2% in Q2 2026 after a neutral clinical trial in the U.K. on high-risk PCI made physicians more cautious about using its Impella pumps, and management expects the rebound to arrive as 2026 turns into 2027.

The rest of the sector also had a good year. Merck returned 87.0% over the same window and Eli Lilly 58.2%, though Pfizer managed 24.4%, so the market paid up for large-cap pharma selectively, and Johnson & Johnson was not the only name it paid for.

You Are Still Waiting For The Talc Agreement To Close

One development over these twelve months was Johnson & Johnson’s alone. In late July 2026, it announced a proposed $5.5 billion settlement framework aimed at resolving the vast majority of pending ovarian cancer talc claims. That is company-specific in a way the peer returns are not. It is also not finished.

The agreement is conditioned, among other things, on the express participation of at least 95% of the remaining claims. A month after it was announced, a Louisiana jury returned a $10 million verdict in a separate talc-related mesothelioma case, in which Johnson & Johnson’s apportioned share was over $1.2 million. Meanwhile, whether the ovarian agreement holds will hinge on reaching that 95% participation threshold.
