Intuitive Surgical (ISRG) stock trades at 48.3 times its earnings over the four quarters to June, against 23.0 for the S&P 500. Shareholders paying that price need to know what they are buying, and management’s message has shifted. During the fiscal Q4 2024 call in January 2025, executives focused on the first phase of the da Vinci 5 launch, a new model of its surgical robot. So what does management talk up now?

Intuitive Surgical Now Talks Up Lower Instrument Prices

During the fiscal Q3 2026 call on October 20, 2026, management argued for pricing actions on a subset of high-volume benign procedures, routine operations such as hernia repair. The company said it wants to lower customer costs where they will have the greatest impact on adoption.

Management measures these actions in da Vinci instrument and accessory revenue per procedure, which tracks what the company earns from instruments and accessories on each da Vinci operation. That figure stood at about $1,860 in fiscal Q3 2026, compared with $1,800 a year earlier. Executives expect the new pricing and a price cut in China to lower that amount by approximately 3% to 5% once fully implemented, holding procedure and product mix steady. They said wider use of da Vinci 5 and Force Feedback is left out of that estimate and should add to the figure.

That is a different case from the one management made in 2025. During the fiscal Q1 2025 call in April 2025, the company called Force Feedback one of the core features of the da Vinci 5. The emphasis has moved from what the new robot can do to what customers pay for each routine operation.

How Big Are Instruments And Systems At Intuitive Surgical?

Instruments and accessories generated 59.8% of Intuitive Surgical’s revenue in fiscal 2025, more than twice the 24.6% from systems, the robots themselves. Systems grew faster, however, with sales rising 25.8% in fiscal 2025 against 18.5% for instruments and accessories.

The latest quarter shows the same pattern. By management’s figures, systems revenue grew 30% in fiscal Q3 2026, while instrument and accessory revenue grew 18%.

The da Vinci 5 is also no longer just a launch: it accounted for 315 of the 546 da Vinci systems Intuitive Surgical placed in fiscal Q3 2026. Yet the lower prices fall on instruments and accessories. This is the business that brings in most of the revenue and grows more slowly than systems.

Should Intuitive Surgical Holders Worry About Lower Prices?

For now, the lower prices appear more concerning than reassuring for holders, because the price cuts on the biggest business are already set, while the extra operations they are meant to win remain management’s expectation. Those cuts start soon. Management said the China price reduction takes effect in the fourth quarter, and extended life instruments, which form part of the broader instrument and accessory pricing actions, roll out by country beginning in the first half of 2027.

Growth in operations is also uneven. During fiscal Q3 2026, da Vinci procedures grew 10% in the U.S., against 20% outside the U.S. In international markets, management said da Vinci adoption for benign procedures grew 41% in the quarter, with hernia repair among the fastest growers, before any of the price cuts take effect. That is the reassuring part: routine operations, where management has focused its price cuts, were already growing quickly abroad.

Management also plans to update its estimate of the long-term opportunity in soft tissue surgery in January 2027, citing its growing investment outside the U.S. and a planned expansion into new countries. Da Vinci instrument and accessory revenue per procedure remains the number to watch when Intuitive Surgical reports its fiscal Q4 2026 results, expected in January 2027.
