Is Johnson & Johnson Stock Priced For More Growth Than It Has Guided?
Johnson & Johnson (JNJ) has had a good year. The stock is up 56.5% over the past twelve months against 19.1% for the S&P 500, and at $271.19 sits at about 99% of its 52-week high. The business is still growing. The risk is that the current stock price discounts an acceleration well ahead of management’s near-term guidance.

Johnson & Johnson Is Not Guiding The Growth You Are Paying For
Against peers, the stock’s earnings multiple of 6.7 looks modest, but measured against J&J’s own history, both earnings and sales trade near multi-year highs—leaving the valuation unusually dependent on faster top-line growth.
Under our valuation model, justifying today’s price over a three-year horizon implies revenue growth of roughly 11.9% a year—even after factoring in significant margin expansion. On the second-quarter call management guided full-year 2026 reported sales growth of 7.0% to 7.6%. Revenue over the last twelve months was $97.9 billion, up 8.1% year over year. That is strong for a company this size, and a long way from 11.9%.
The 2026 guide sits well below the 12% annualized growth required over the next three years, leaving management’s longer-term target of double-digit growth by the end of the decade to do the heavy lifting later. Nothing has to go wrong for that gap to matter. It only closes if both halves of the company grow into it.

And Only One Side Of The Company Is Growing Fast
In the second quarter of 2026 the company posted $25.3 billion, up 5.6% operationally. STELARA alone cost about 460 basis points of that growth, its sales down 55.7% as biosimilars took share; excluding STELARA the company grew double digits. Covering that hole falls to a short list of newer medicines. TREMFYA grew 71% and DARZALEX again delivered more than $4 billion, while ICOTYDE in psoriasis and INLEXZO in bladder cancer are still in their infancy, management says.
The other side is slower. MedTech posted $8.9 billion in the same quarter and grew 3.6% operationally, roughly half the 6.8% operational pace of the medicines business. The CFO’s own account was that cardiovascular results were not to the company’s standards, primarily in heart recovery.
Abiomed sales fell 2% after physicians turned more selective on Impella, its heart pump, following a neutral clinical trial in the U.K. The company’s answer is PROTECT IV, a trial expected to read out in 2027. Management expects more modest growth rather than declines until that read-out, then gradual improvement back to double-digit growth.
So What Would You Actually See Go Wrong First?
Probably nothing you could point to on any given morning. The likeliest version of this risk is the company still growing revenue at the trailing 8% while the price keeps asking for 12% a year, and the multiple drifting down to meet it. That takes quarters, and it looks like good news most of the way.
Management has said MedTech should grow better in the second half of 2026 than in the first. If that acceleration arrives, the weight on a few launches lightens; if not, more of the load sits on TREMFYA and products barely out of the gate. Implied volatility on the stock, at 22, sits in the 69th percentile of its own past year, so the size of the move the options market is pricing already runs above its own normal.
