Intuitive Surgical (ISRG) has handed its owners a hard year. Management cited the law of large numbers alongside deferrable procedures, noting that the underlying disease burden is unchanged and patients eventually return, while CEO Rosa also relayed feedback from some customers that insurance coverage changes may be having an effect. An analyst pushed back, noting other med tech companies are not seeing the same effect.

GLP-1 use is shrinking US da Vinci bariatric cases at the same time. From the first half of 2027 the extended use program raises the number of uses on a subset of instruments, cutting what customers pay per procedure to support broader adoption where cost constrains it, and management has not put a number on the revenue impact yet. Customers are still taking the robots: da Vinci system placements rose 18% year over year in Q2 2026 on demand for da Vinci 5.

Size For The $240 Floor Before You Argue About The Ceiling

None of this says the business is coming apart. Revenue grew 20.7% over the trailing twelve months and the operating margin sits at a three-year peak. What the premium prices is how long the US procedure question and the instrument cost reset take to settle, and management describes that reset as a progressive impact through 2027 instead of a single step.

So the decision is about size, and the level to test yourself against is $240. A holder comfortable now who could not live with the stock sitting below $236 a year from now is carrying too much, because the market puts roughly a 16% chance on exactly that.
