Why Is UNH Stock Trading At A Premium With The Slowest Growth?
UnitedHealth (UNH) runs the best operating margin of the three managed care companies in its peer group, and the slowest revenue growth of the three. It also carries a significant valuation premium over its peer, Cigna, trading at more than double its earnings multiple.

Where Does UnitedHealth Actually Rank In Its Own Group?
The size of the premium is clearest against Cigna. UnitedHealth trades at 25.6 times trailing net earnings on a 4.8% operating margin—though its raised 2026 guidance brings that multiple closer to 21 times forward earnings. Cigna trades at 11.3 times trailing net earnings on 3.2%. CVS trades at 25.1 times, but on trailing earnings that include at least one loss quarter, so it does not compare with a clean year.

The premium is not buying revenue growth. UnitedHealth’s revenue grew 6.5% over the last twelve months, the slowest in the group, against Cigna’s 7.7%. Nor is the premium buying the group’s highest trailing return—CVS delivered 34.3% against UnitedHealth’s 30.5%—even as both soundly beat the S&P 500’s managed care index over the same stretch. The stock also sits about 33% below its two-year high.
How Does UnitedHealth Earn That Margin?
Not only in the health plan. UnitedHealthcare is guided to at least $12 billion of operating earnings for 2026, against about $25.45 billion guided for the whole company. Revenue over the trailing twelve months was $450.13 billion, the base the 4.8% trailing margin is measured against, and the 2026 guidance is what shows where those earnings come from. The lead is earned beside the insurance book as well as inside it.
The rest sits in Optum. Optum Rx, the pharmacy benefit business, is guided to $6.25 billion of operating earnings for 2026 on its own, and management has been rebuilding it around monthly per-member fees. Management expects to end 2026 with more than 95% of clients on full pass-through of manufacturer rebates. Optum Health, the care delivery arm, cares for 20 million people through primary and specialist care, ambulatory surgery and home health.
What Has To Hold For You To Pay Up?
Management raised its 2026 adjusted earnings per share guidance to a range of $19.50 to $20.00, up from greater than $18.25. The test is the second half of 2026, now underway, when earnings tilt heavily toward Optum Rx and Optum Insight to make up for UnitedHealthcare’s front-loaded year.
The pressure sits in the commercial book. Management says medical cost trends there are running modestly above 11%, higher than planned, and full commercial margin recovery to its target 7%-plus range remains pushed past 2027, as management previously indicated. One driver is the arbitration process under the No Surprises Act, which management says is adding about 50 basis points of incremental cost trend in 2026, with management noting that roughly 60% of cases are driven by just five entities.
UnitedHealthcare is opening its front door wider at the same time. From October 1 a broad range of conditions no longer needs prior approval, part of a plan to eliminate 30% of its prior authorization volume by the end of 2026.
